You mean by the END of Dynasty 5. There is no evidence before that on which to base such an assertion.

Yes, this came out of the great Fusion of Dynasty V... nothing indicates it has any roots before that.

No, the first attestation is at most 50 years before Unas. 250 years are still "centuries".
