Of course, there must have been a plan in one or another form, but I believe not as detailed as you apparantly like to think. In my opnion, as far as construction is concerned, the AE had to plan much more in detail when the level of the QC-floor was reached. Once the roof of the KC was placed, planning became less intense again. Imo, you don’t really need detailed plans to lay a course, even when courses are ‘interrupted’ by cavities (corridors, chambers, shafts). Only when your workers reach the center of a horizontal layer (thus the center of the pyramid) where their blocks come in the immediate vicinity of e.g. a chamber, your architects need to plan much more in detail.

The ratio rise/horizontal run I believe was used for the slopes of pyramids, but not for the shafts.

I believe the shafts were not built on purpose, I believe they were not planned in advance, because only the GP has such shafts. But that is only an opinion. It was not a characteristic feature of 4th Dynasty pyramid-building to make such unique shafts, neither during later Dynasties. I believe Khufu was an impulsive builder, that is why his tomb is so unique. I believe the ‘orientation’ of the shafts is random (so, no seked used) ; the wobbling and curling seem to indicate this, and also that they were designed not to let something out but to allow something to come in.

No, because I am convinced that, if Khufu died during construction of the QC, the KC would not have existed.
