I read, with a great deal of amusement, a comment on Pharyngula from someone who HAD seen and handled the original document:

I am quite sure I got the only copy in existence when PU (Patriot University) mailed it to me because it came with a picture cut out of a magazine and affixed to page 79 or so with transparent tape. I mean, who does that in a real dissertation?
